The Nursing-Transportation Partnership (NTP) aims to increase access to health and mobility and address healthcare gaps through a transportation and mobility lens.
NTP is an extension of our pilot program called 25 for 80 non-emergency medical transportation (NEMT) project. Through 25 for 80, we learned the vital roles transportation and mobility play in accessing care. In southern Delaware, transportation options are limited. 90% of the population use their own cars to accessing care. Long distance care is common with many new residents tend to travel back to providers and specialists from their former neighborhoods. Western Sussex County of Delaware is considered a medical desert with many residents living in rural areas with limited access to care and transportation.
In the coming years, people age 80+ will be the fastest growing population in southern Delaware. The need for transportation and mobility solutions will be more urgent than ever. It's our goal to use our collective nursing experience to become a partner in creating real and viable transportation and mobility solutions with other concerned organizations and neighbors.
Nurses have always been involved in transportation. It's not uncommon for a charge nurse in a hospital to coordinate safe patient transport based on the nursing process. Discharge nurses, emergence room nurses, flight nurses, home health nurses, oncology nurses and hemodialyisis nurses are often involved in transportation coordination so that they can provide safe patient care and transfer.
In addition to our 25 for 80 NEMT project, we are developing a volunteer chaperone program in partnerhsip with Trusted Riders, building a database of transportation options to local and long distance care sites using generative artificial intelligence, working with public transportation to address paratransit for people with disabilities, adopting a public transportation shelter, and founding the Delaware Health and Mobility Alliance.
We thank the generous support from AARP for choosing Nurses & Neighors as a 2023 Community Challenge grantee. We appreciate the trust and donation from the Ladies Ancient Order of Hibernians of Sussex County for their seed funding for our volunteer chaperone training program. We thank our ongoing partners DART paratransit and Trusted Riders. Special thank you to our donors who understand the need for real transportation solutions and trust in their nurses to make that happen!
